YF&R Toolkit Supply Form



We look forward to helping you reserve the items your committee needs to make your event the most memorable! As stated in the toolkit, this order form needs to be completed 90 days in advance of the day your event is scheduled to start. If any of the toolkit items you reserve are already checked out for the date specified on this form, you will receive notification from your District Field Representative.

Once reserved, your District Field Representative will work with you and the state office in order to get the items to your county prior to the event. Following your event, please work with your District Field Representative to return the items in a timely manner.

*Please note: Any materials checked out from the state office will be limited to 21 days of total possession per event. At the discretion of the state office, if items are damaged, the cost of repair or replacement shall be the responsibility of the county YF&R group that borrowed them.

Please note: The * notates that this item will need to be ordered ahead of time by the county YF&R committee using the vendor of their choice. Once this reservation form is submitted, the individual who submitted the form will receive an email within 3 business days from your District Field Representative with the attachment of the design work. (Pro tip: A good practice when working with print shops is to request a proof to approve prior to purchasing the finished product.)
